• When DHA processes your request, they will provide you with the necessary documents
• Original passport / clear passport copy / identity card with reference number (mandatory)
• Permission copy of residence
• Presently active telephone number
• Physical presence of the applicant (If your visa status changes, you'll need to fill out a Change of Status form.)

Flash points to remember

• After submitting the service fees, this application will not be cancelled.
• Please retain the application form and show it at any counter if necessary.
• Please use a recent clear personal photo with a white backdrop that is no older than three months. Scanner-generated images will not be allowed.
• Please wear a cotton shirt with no buttons or graphics to speed up the x-ray procedure for the male category.
• From the date of issue, this Medical Fitness certificate can be used for three (3) months.
• Please return to the medical fitness division if you have not received your MF accreditation within five working days.
• For new visas They have to attach employment visa or change of status Original passport

For the below six groups, HBV vaccination is required and will be invoiced individually at a rate of AED 40:

• Nanny
• Housemaids and other comparable occupations.
• Supervisors at the nursery and kindergarten.
• Employees in hair salons, beauty salons, and health clubs.
• Food handlers, processors, and agencies in charge of food safety.
• Cafeteria and restaurant employees.
